The novel's central theme is the death of Dolores's husband, Joe, a violent alcoholic who abused her and their daughter. Dolores confesses that she killed him to protect her daughter, pushing him down a well, with Vera's tacit approval.

The novel has no chapters or transitions, but flows like one long monologue, creating a sense of intense intimacy and realism. Through Dolores's raw and emotional confession, King explores themes of domestic violence, female strength, justice, and guilt.
